I voluntarily agree to participate in the documentary project concerning the history, architecture, restoration, and legacy of 1206 West Louisiana Street.

1. Permission to Record and Use My Participation

I authorize MAAE Media / Michael Lewis to photograph, film, record, edit, reproduce, and use my image, likeness, name, voice, interviews, statements, and other contributions made during my participation.

This permission extends to the completed documentary, promotional excerpts, trailers, social media, websites, property marketing, historical and educational presentations, public screenings, and other distribution directly related to the project.

2. Editing and Creative Control

I understand that my recorded participation may be edited for length, clarity, pacing, and storytelling.

I acknowledge that MAAE Media retains creative and editorial control over the production and that my participation does not guarantee inclusion in the completed film.

I do not require prior approval of the final documentary or its related promotional materials, provided my contributions are not knowingly presented in a materially false or misleading manner.

3. Archival Materials and Historical Information

If I provide photographs, videos, recordings, documents, or other historical materials, I authorize MAAE Media to reproduce and use those materials in connection with this project to the extent that I own or control the necessary rights.

I understand that providing an original or copy of a historical item does not necessarily establish copyright ownership.

I agree to disclose any known third-party ownership, restrictions, or attribution requirements.

I understand that historical information and personal recollections I provide may be reviewed, researched, verified, and incorporated into the documentary at the producer's editorial discretion.

4. Compensation and Ownership

I understand that participation in this documentary is voluntary and that I will not receive compensation, royalties, or ownership rights in the completed production.

This release does not transfer ownership of any independently owned archival materials I contribute.

5. Duration of Permission

The permissions granted under this release are worldwide, non-exclusive, royalty-free, and perpetual for uses connected to this documentary, its promotion, exhibition, archival preservation, and related historical or educational presentation.

This release does not authorize the use of my likeness or voice to falsely imply endorsement of an unrelated product, service, or organization.

6. Acknowledgment and Consent

I have read and understand this release and voluntarily authorize the uses described above.
